Silsbee Oaks Health Care, L.L.P. provides long-term health care, which attends to the emotional, mental, physical, social and spiritual well being of the residents.
We utilize interdisciplinary clinical teams that include physicians, nurses, therapists, dietitians, social workers and case managers to plan and deliver specialized programs of care.

Silsbee Oaks Health Care, L.L.P. is a 160 bed facility specializing in the provision of short-term inpatient and outpatient rehabilitation services and long-term care.

The quality care we provide is accomplished by staff that is trained to care for the individual needs of each resident. We provide care according to each resident's particular functioning level, ranging from early dementia to medically complex needs.
Care of the resident is directed by an attending physician.
A Licensed Registered Dietitian conducts ongoing assessments of each resident's nutritional status. Specialized diet plans are provided as appropriate.
Silsbee Oaks Health Care L.L.P. a skilled nursing facility is certified for Medicare and Medicaid, private and Hospice.
Social Services
The Social Services Department facilitates admissions to Silsbee Oaks Health Care, L.L.P., identifies the psychosocial needs of the residents, assists in helping residents achieve their highest potential and facilitates discharge planning. Social Services staff works with all departments to develop a care plan to assist residents in achieving their goals, which often include returning to their home or lesser level of care following rehabilitation.
