The Snyder Oaks Care Center was established on 37th Street in Snyder, Texas in 1977. In 1998, Care Inn Properties, Inc. acquired ownership and management of our highly regarded community. The compassionate care and concern we extend to our residents and families are reflective of our shared history that spans nearly 30 years. We are proud to offer the most premier services available today, with an expert staff dedicated to serving others. We understand that serving our residents is an opportunity to care for our friends and neighbors in our community. In 2005, Care Inn Properties changed its name to Touchstone Communities. So now, the Snyder Oaks Care Center, with a rich history of caring for so many so well throughout the years, continues this reputable tradition and is now also known as a Touchstone Community.
Rehabilitation
Snyder Oaks Care Center provides comprehensive Rehabilitation Services consisting of Speech, Occupational and Physical Therapies administered by state licensed professionals under the supervision of a Physician. Our Rehabilitation Team focuses on a "Return to Home" approach, designing a customized program of physical medicine that provides the highest opportunity to return to their prior level of function and living accommodations. Our therapists are specially trained in the treatment of fractured hips, hip replacements, knee replacements, strokes, complicated medical situations and general re-conditioning. Physical Therapists work with gross motor skills such as walking, wheelchair training, transfer training and balance skills, to name a few. Occupational Therapists work with activities of daily living, such as bathing, dressing, homemaking, memory and community reintegration skills. Speech Therapists work with communication skills, both speech and cognitive issues, and swallowing dysfunction as a result of a stroke or accident. Restorative Nursing works with individuals after active rehabilitation therapy to ensure the patient does not regress from the skills they gained from therapy. Our goal is to get our patients back to their highest practical level of function.
